Terms of Service

I understand that this procedure requires single synthetic eyelashes to be adhered to my own natural eyelashes.

I understand that it is my responsibility to keep my eyes closed and be still during the entire procedure, until my lash artist addresses me to open my eyes.

​

I understand that some risks of this procedure may be, but not limited to eye redness and irritation due to the vapors and fumes from the adhesive.

​

I agree to disclose any allergies or other topics my service provider should be aware of.

​

I agree to only use products supplied by Gem Salon & Boutique Limited and eyelash extensions will not be guaranteed with any other products.

​

I agree that by reading and signing this consent form, I release Gem Salon & Boutique Limited from any claims or damages of any nature.

I agree that I read and fully understand this entire consent form.

​

I am of sound mind and capable of executing this wavier for myself.

​

I give Gem Salon & Boutique Limited permission to show my before and after photos of eyelashes to other potential clients. 

​

I have read and completed the consent form in its entirety and have answered everything to the best of my ability. I have been informed of potentially harmful or negative side effects that could be caused by the application and/or removal process.

I confirm and agree that I wish to engage the services of Gem Salon & Boutique Limited.

​

I understand that if I am over 15 minutes late to my appointment this will be considered a No-Show and charged a Cancellation Fee.

​

I agree that If I fail to cancel or reschedule 24 hours before my appointment Gem Salon & Boutique can charge my card on file for a cancellation fee.

​

I understand the cancellation fee is $50 for all services.

​

Cancellation policy is fully agreed and understood.
